For the first question, the main subject of the sentence is the book. Because of that, your description (written by Mark Twain) comes after your introduction of the subject (The Adventures of Huckleberry Finn).

For the second question, the main subject of the sentence is Betsy Read. So your decription of her as a "famous piano player" or "Sam's girlfriend" can be swapped as long as it comes after her name.
